- Heart rhythm disorders
- Myasthenia gravis
- Optic neuritis
- Quinine, quinidine, or mefloquine sensitivity
- History of thrombocytopenia or unusual bleeding reactions
- Severe kidney or liver impairment
- Pregnancy or breastfeeding unless medically supervised
- G6PD deficiency or history of hemolytic anemia
